Payment Options

Take Shape Surgery Center L.L.C. wants to assist you in meeting your healthcare financial needs in the best possible way. For your convenience, we accept MasterCard, VISA, American Express, and Discover credit cards. We also accept personal checks if payment is rendered two weeks before your scheduled procedure, cash, cashier’s checks, money orders, and CareCredit.

The estimate of charges will be based on procedure and duration.

Facility Fee Covers: The facility has one fee that covers the following items: nursing, technician and related services; use of the facility; testing for certain lab tests performed at the surgery center such as glucose(blood sugar), and pregnancy; medications administered before, during and after your surgery while in the facility; surgical supplies used by the physician’s and staff; equipment used in the facility; surgical dressings and PACU recovery time up to 2 hours.

Pre-treatment estimates for non-emergency services are available upon request. Patients and prospective patients may request from the facility and other health care providers a personalized estimate of charges and other information.  Services may be provided by the facility and other health care providers who bill separately. The other health care providers may or may not participate with the same insurers or health maintenance organizations as the facility. This information is available for each facility below.

Fees for your physician, surgeon, radiologist, anesthesiologist, and pathologist’s services will not be included in your bill. The Surgical Center independently contracts these specially trained physicians to provide specialty services.
